Agenda

This session builds the documentation foundation that every neoplasm code rests on, teaching coders to distinguish primary, secondary (metastatic), and personal history of malignancy and to recognize exactly what the provider must state to support each. Rather than presenting Chapter 2 guidelines in isolation, we work them inside real oncology scenarios, beginning with the common question of how to handle cancer cells identified at a second site. We also practice reading an oncology note and decoding its abbreviations so the coder can work confidently from the record in front of them. Attendees leave able to select and sequence the correct neoplasm code and to write a clean query when the note falls short.

Learning Objectives

  1. Differentiate primary, secondary (metastatic), and personal history of malignancy, and identify the specific documentation elements required to support each status.
  2. Apply ICD-10-CM Chapter 2 guidelines to select and sequence neoplasm codes based on the reason for the encounter and the treatment status of each site.
  3. Interpret the structure and common abbreviations of an oncology note, and recognize when documentation is insufficient and a provider query is required.
