CMS developed new HCPCS codes for primary care practitioners who provide care management services and communications-technology based services on a monthly basis. These are an alternative to other care management codes, and acknowledge that much of the work caring for patients occurs outside of regular office visits. These Advanced Primary Care Management (APCM) codes are for use by primary care practitioners who are the focal point for all needed health care services for the patient. The service may be billed by one practitioner per month for each beneficiary by the practitioner who is the continuing focal point for all needed health care services and responsible for all primary care services for the patient.
