This is the third of three in-depth webinars on auditing E/M services.  Each webinar will cover one of the elements of medical decision making.

The risk of additional diagnostic testing and management is distinct from the risk of the condition itself. Risk also includes the decision to forgo treatment, but what is the best way to document that? This session will describe the best practices in documenting the condition, the status of the condition and the plan of treatment. It will provide guidance on what needs to be documented to credit prescription drug management and if and how over-the-counter medication can carry moderate risk. It will also describe what the AMA says about the risk of a procedure related to patient risk and the procedure itself.

  After the session, participants will be able to:

  • Give an example of the documentation needed to support the decision to forgo treatment
  • Explain the difference risk related to a procedure itself and risk related to an individual patient
  • Consistently audit E/M notes related to patient management
