CMS is implementing a mandatory, disease-specific reporting model beginning in the CY 2027, reporting due March 31, 2028. Who will be required to report?  And for what conditions?

Physicians who work in the selected geographic areas listed in this excel sheet, downloaded from CMS’s website under Participant Information, “ASM Mandatory Geographic Areas.” The next file down lists individual physicians who are included so you can search for your physicians’ names. CMS will add names to this list based on claim submissions in 2025. Non-physician practitioners and physicians who work exclusively in RHCs or FQHCs are not required to particpate.
