CMS developed three HCPCS codes effective 1-1-2025 to pay primary care practices and manage the care of patients over time. CMS continues to believe that practices incur significant expenses for the care that happens in between office visits. This webinar will describe the three codes and associated levels, the required capabilities of the practice, the services that may not be reported in the same calendar month, and reporting requirements.
After the webinar, participants will be able to:
- Describe the consent required to provide the services
- Assess if their practice has the required capabilities to perform APCM
- List the services that may not be reported in the same calendar month

This program has the prior approval of AAPC for 1 continuing education credit. Granting of prior approval in no way constitutes endorsement by AAPC of the program content or the program sponsor.
