Codes 99453, 99445 and 99454 are for the initial set up and patient education on use of equipment, device supply with daily recordings.
99091 describes a physician or other qualified health care professional collecting and interpretating physiological data and treatment management by a physician or other qualified healthcare professional.
99473 describes validating a blood pressure cuff for accuracy and providing patient education training and device calibration so that a patient can take their own blood pressure at home.
99474 describes reviewing the measurements (with certain minimum readings) and communicating a treatment plan to the patient.
