CDI and Coding of Heart Failure | Webinar
Heart failure coding is challenging because there are multiple classification systems, and getting the type and acuity right can swing both CC/MCC status and HCC risk adjustment significantly. Coders and CDI staff often see documentation that labels a patient as having “heart failure” without the clinical specificity needed to support accurate diagnosis coding. This webinar will discuss the gap between clinical documentation and coding requirements with a goal of decreasing audit risk and submitting claims with accurate, defensible diagnosis codes.
Learning Objectives:
By the end of this session, participants will be able to:
- Differentiate the heart failure classification systems and its CC/MCC and HCC impact.
- Apply the clinical criteria that validate acute (decompensated) heart failure to distinguish a codeable acute exacerbation from documentation that only reflects a chronic or unsupported diagnosis.
- Identify high-yield query and audit opportunities in HF charts so the diagnosis can be appropriately queried for or challenged.
